Business Licenses
This question pertains to professional, occupational and business licenses other than those to practice law. This set of questions asks for all such licenses held, 24(b).
Question subpart 24(a) concerns instances of issuance denials or negative action, whether or not the record was expunged or sealed.
Question 24(c) inquires about suspensions.
24(d) involves government agency discipline.
Lastly, question 24(e) calls for disclosure of grievances or charges made.
Information regarding the dates the license was held are generally available from the agency providing the license. This set of questions also requires an answer regarding discipline, complaints, suspensions, grievances, and charges whether or not the record was expunged or sealed. If you obtained a local, rather than state regulated license, you should also search in the city or county where the license was obtained. The licenses are now generally referred to as Local Business Tax Receipts, and apply mostly to Businesses rather than "Professional" licenses. The search functions for such local licenses are usually robust for Contractor's Certificates of Competency, but weak for other areas.
