Questions 3c, 6a, and 6b
Question 3(c) asks for your longest residence since age 16. This question only requires the city, state and dates.
Question 6(a) requires the complete address for all of residences for the past 3 years.
Question 6(b) asks for all residences since age 16. This question only requires the city, state and dates.
To locate old addresses, look up:
- SAT, ACT or similar test score reports
- shipping addresses saved in online retailers (Amazon.com)
- high school and undergraduate transcripts and/or report cards
- utility providers; if you had utilities in your name, check with service providers for all of the addresses at which you had service
- tax returns and pay stubs (see Tax Returns tab)
- credit report
Finally, you can check free people search websites. Keep in mind, these websites index only freely accessible public information, so if you did not include your full name in a utility application, or had your information removed from the white pages, or have a "common" name, the results may not be about you.
